Abstract Objectives: to evaluate the association of pre-pregnancy and current body mass index and the density of dietary macronutrients on underreporting of energy intake at pregnancy. Methods: cross-sectional analysis of 327 postpartum women from the city of Mesquita, in Rio de Janeiro. A food frequency questionnaire was administered at maternity ward having the last six months of the pregnancy as the time frame. Energy balance was considered as the outcome, and it was calculated as the division of energy intake by basal metabolic rate (underreport <1.35). Multivariate logistic regression was applied to test the associations between body mass index (pre-gestational and postpartum) on energy balance (underreport or not). Dietary density of protein (4th quartile), carbohydrate (1st quartile) and fat intake (1st quartile) were tested. Results: mean energy intake was 2,894 kcal and near of 25% of the women were considered as underreported during pregnancy. Obese women had higher chance (OR=1.90; CI95%=1.09-3.33) of being underreported at pregnancy. Underreported women presented greater chance of report dietary intake with higher contents of protein (OR=2.37; CI95%=1.37-4.09) and lower density of fat (OR= .81; CI95%=1.04-3.15). Conclusion: underreported pregnant women had higher chance of report great and lower amounts of protein and fat dietary densities.


Resumo Objetivos: avaliar a associação entre o índice de massa corporal pré-gestacional e pós-parto e a densidade de macronutrientes da dieta com o sub-relato de energia da dieta na gestação. Métodos: análise transversal com 327 puérperas da cidade de Mesquita, no Rio de Janeiro. Aplicou-se o questionário de frequência alimentar na primeira semana após o parto tendo como base o consumo dos dois últimos trimestres gestacionais. O balanço energético foi calculado a partir da divisão da ingestão de energia pela taxa metabólica basal (sub-relato<1,35). Adotou-se a regressão logística multivariada para analisar as associações entre os índices de massa corporal e a densidade dos macronutrientes da dieta (proteína, carboidratos e lipídios) com o balanço energético (sub-relato ou não). Resultados: a ingestão média de energia foi de 2.894 kcal e 25% das mulheres foram classificadas com sub-relato. Mulheres obesas no pós-parto tiveram maiores chances (OR=1,90; IC95%=1,09-3,33) de sub-relato de energia na gravidez e gestantes com balanço energético <1,35 apresentaram dieta com maior densidade de proteína (OR=2,37; IC95%=1,37-4,09) e menor densidade de gordura (OR=1,81; IC95%=1,04-3,15). Conclusão: a obesidade no pós-parto foi associada ao sub-relato de energia na gravidez e o balanço energético associou-se a densidade dos macronutrientes da dieta

BACKGROUND: Appetite regulation is integral to food intake and is modulated by complex interactions between internal and external stimuli. Hormonal mechanisms which stimulate or inhibit intake have been characterized, but the physiologic effects of serum levels of such hormones in short-term appetite regulation have received little attention. AIM: To evaluate whether fasting levels of orexigenic/anorexigenic hormones were associated with energy intake at breakfast, served soon after drawing a fasting blood sample, in a group of adolescents. MATERIAL AND METHODS: Anthropometry, body composition and fasting blood levels of leptin, insulin, ghrelin, and orexin-A were measured in 655 Chilean adolescents aged 16.8 ± 0.3 years (52% males). Energy intake was measured at a semi-standardized breakfast. Associations between hormone levels and energy intake were studied using multivariate linear models. RESULTS: Thirty nine percent of participants were overweight/ obese. After an overnight fast, median values for leptin, insulin, ghrelin and orexin-A were 7.3 ng/mL, 6.7 IU/dL, 200.8 pg/mL, and 16.1 pg/mL, respectively. Participants ate on average 637 ± 239 calories at breakfast. In multivariable models, insulin levels were inversely and independently associated with caloric intake at breakfast (β = −18.65; p < 0.05), whereas leptin, ghrelin and orexin-A levels were positively and independently associated with intake: β= 5.56, β = 0.34 and β = 8.40, respectively, p < 0.05. CONCLUSIONS: Fasting leptin, ghrelin and orexin-A were positively associated with energy intake during breakfast provided soon after the blood draw. Insulin was negatively associated with energy intake. Modifiable factors influencing levels of appetite regulating hormones could be a potential target for influencing food intake.


ANTECEDENTES: La regulación del apetito es parte integral de la ingesta alimentaria y es modulada por complejas interacciones entre estímulos internos y externos. Se han caracterizado los mecanismos hormonales que estimulan o inhiben la ingesta, pero los efectos fisiológicos de los niveles séricos de tales hormonas en la regulación del apetito a corto plazo han recibido poca atención. OBJETIVO: Evaluar si los niveles en ayunas de hormonas orexigénicas/ anorexigénicas se asocian con la ingesta energética en el desayuno, entregado inmediatamente después de una muestra de sangre en ayunas, en un grupo de adolescentes. MATERIAL Y MÉTODO: Se efectuaron mediciones antropométricas, composición corporal y medición de niveles en ayunas de leptina, insulina, grelina y orexina-A en 655 adolescentes de 16,8 ± 0,26 años. La ingesta energética se midió en un desayuno semiestandarizado. Se estudiaron las asociaciones entre los niveles hormonales y la ingesta energética mediante modelos lineales multivariados. RESULTADOS: Los valores de leptina, insulina, grelina y orexina-A fueron 7,3 ng/mL, 6,7 UI/dL, 200,8 pg/mL y 16,1 pg/mL respectivamente. Los participantes comieron un promedio de 637 ± 239 calorías en el desayuno. Los niveles de insulina se asociaron inversa e independientemente con la ingesta del desayuno (β = −18,65; p < 0,05), mientras que los niveles de leptina, grelina y orexina-A se asociaron positiva e independientemente con la ingesta: β = 5,65; β = 0,34; β = 8,40, (p < 0,05). CONCLUSIONES: La leptina, grelina y orexina-A en ayunas se asociaron positivamente con la ingesta de energía durante el desayuno proporcionado poco después de la muestra de sangre. La insulina se asoció negativamente con la ingesta de energía. Los factores modificables que influyen en las hormonas reguladoras del apetito podrían ser un objetivo potencial para influir en la ingesta de alimentos.

ABSTRACT

Abstract In a healthy athlete, the caloric intake is sufficient for sports energy needs and body physiological functions, allowing a balance between energy availability, bone metabolism, andmenstrual cycle.Onthe other hand, an imbalance causedby low energy availability dueto a restrictive diet, eating disorders or long periods of energy expenditure leads to multisystemic deregulation favoring the essential functions of the body. This phenomenon, described as the female athlete triad, occurs in a considerable percentage of high-performance athletes, with harmful consequences for their future. The present review was carried out based on a critical analysis of themost recent publications available and aims to provide a global perception of the topic relative energy deficit in sport (RED-S). The objective is to promote theacquisition ofmore consolidated knowledgeon an undervaluedtheme, enabling the acquisition of preventive strategies, early diagnosis and/or appropriate treatment.


Resumo Emumaatleta saudável, oaporte calórico é suficientepara anecessidade energética esportiva e para as funções fisiológicas corporais, permitindo um equilíbrio entre disponibilidade energética (DE), metabolismo ósseo e função menstrual. Por outro lado, um desequilíbrio devido à baixa disponibilidade energética (BDE) por dieta restritiva, perturbações alimentares ou grandes períodos de gasto energético conduz a uma desregulação multissistêmica priorizando as funções essenciais do corpo. Este fenômeno, descrito inicialmente como tríade da mulher atleta e, atualmente, comodéfice energético relativo no esporte (RED-S, nasigla eminglês) tem como pilares a BDE, disfunção menstrual e alterações na densidade mineral óssea (DMO), estando presente em uma percentagem considerável de atletas de alta competição, com consequências nefastas para o seu futuro a curto, médio e longo prazo. A presente revisão foi realizada a partir da análise crítica das publicações mais recentes disponíveis e pretende proporcionar uma percepção global do tema RED-S. O objetivo é promover a aquisição de um conhecimento mais consolidado sobre uma temática subvalorizada, possibilitando a aquisição de estratégias preventivas, diagnóstico precoce e/ou tratamento adequado.

Abstract Aim: To evaluate the intensity of training and official badminton matches (international championship) in junior elite players. Methods: Twelve elite players from the Brazilian junior national team (6 male and 6 female) were monitored during 19 training sessions and 50 official men's and women's single and double matches in the XXVII Pan-American Junior Games. Subjects underwent an incremental running step test to determine maximal oxygen uptake (VO2max), and an individual relationship between heart rate (HR) and VO2max was established to estimate exercise intensity and oxygen uptake at official matches (OMs) and training sessions. HR was monitored during multi-shuttlecock (MS), technical-tactical (TT), and physical (PS) training methods, as well as during simulated matches (SMs) and OMs. Variables such as %HRmax, HR zones, %VO2max, and energy expenditure were also calculated. Results: OM was the most intense activity monitored for male and female athletes, followed by SM according to %HRmax (86.8 ± 4.1% and 84.4 ± 5.0% for female and male, respectively, in OM vs. 74.6 ± 2.3% and 75.0 ± 5.0% for female and male, respectively, in SM). OM mean energy expenditure was 10.7 ± 0.5 kcal.min-1 for females and 14.9 ± 4.6 kcal.min−1 for males. Conclusion: MS training has less physiological demand and OM presented higher intensity. SM, on the other hand, had the closest physiological demand to an OM.

ABSTRACT Objective: To perform anthropometric and dietary evaluation of patients with glycogenosis type Ia and Ib. Methods: This cross-sectional study is composed of a sample of 11 patients with glycogenosis divided into two subgroups according to the classification of glycogenosis (type Ia=5 and type Ib=6), aged between 4 and 20 years. The analyzed anthropometric variables were weight, height, body mass index, and measures of lean and fat body mass, which were compared with reference values. For dietary assessment, a food frequency questionnaire was used to calculate energy and macronutrients intake as well as the amount of raw cornstarch consumed. Mann-Whitney U test and Fisher's exact test were performed, considering a significance level of 5%. Results: Patients ingested raw cornstarch in the amount of 0.49 to 1.34 g/kg/dose at a frequency of six times a day, which is lower than recommended (1.75-2.50 g/kg/dose, four times a day). The amount of energy intake was, on average, 50% higher than energy requirements; however, carbohydrate intake was below the adequacy percentage in 5/11 patients. Short stature was found in 4/10 patients; obesity, in 3/11; and muscle mass deficit, in 7/11. There were no statistical differences between the subgroups. Conclusions: In patients with glycogenosis type I, there was deficit in growth and muscle mass, but no differences were found between the subgroups (Ia and Ib). Although the diet did not exceed the adequacy of carbohydrates, about 1/3 of the patients presented obesity, probably due to higher energy intake.


RESUMO Objetivo: Realizar avaliação antropométrica e dietética de pacientes com glicogenose tipos Ia e Ib. Métodos: Estudo transversal composto de uma amostra de 11 pacientes com glicogenose divididos em dois subgrupos de acordo com a classificação da glicogenose (tipo Ia=5; tipo Ib=6), com idades entre 4 e 20 anos. As variáveis antropométricas analisadas foram peso, estatura, índice de massa corporal e medidas de massa magra e gorda, que foram comparadas com valores de referência. Para avaliação dietética, foi utilizado um questionário de frequência alimentar para cálculo de ingestão de energia e macronutrientes, além da quantidade de amido cru ingerida. Realizaram-se testes U de Mann-Whitney e exato de Fisher, com nível de significância de 5%. Resultados: Os pacientes ingeriram amido cru na quantidade de 0,49 a 1,34 g/kg/dose na frequência de seis vezes ao dia, inferior à dosagem preconizada (1,75-2,50 g/kg/dose quatro vezes ao dia). A quantidade de energia consumida foi, em média, 50% a mais que as necessidades, contudo o consumo de carboidratos foi abaixo da porcentagem de adequação em 5/11 pacientes. Baixa estatura ocorreu em 4/10 pacientes, obesidade em 3/11 e déficit de massa muscular em 7/11. Não houve diferença estatística entre os subgrupos. Conclusões: Em pacientes com glicogenose tipo I, houve déficit de crescimento e de massa muscular, mas não diferença significante entre os subgrupos (Ia e Ib). Embora a dieta não tenha ultrapassado a adequação de carboidratos, 1/3 dos pacientes apresentou obesidade, provavelmente pela maior ingestão de energia.

ABSTRACT Objective To evaluate the nutritional risk factors in patients eligible for hematopoietic stem cell transplantation. Methods A cross-sectional, descriptive study conducted with patients recruited from an hematology outpatient clinic. Study variables included demographic and clinical data, patient-generated global subjective assessment findings, anthropometric indicators, food intake and oxidative stress levels. The level of significance was set at 5% (p<0.05). Results The sample comprised 72 patients, mean age of 48.93 years (14.5%). Multiple myeloma was the most prevalent condition (51.4%) in this sample. Most patients (55.6%) were overweight according to body mass index and at risk of cardiovascular disease according to waist circumference, conicity index and percentage of body fat. Sarcopenia was associated with risk of cardiovascular disease, hip-to-waist ratio (p=0.021), muscle strength depletion (p<0.001), food intake (p=0.023), reduced functional capacity (p=0.048), self-reported well-nourished status; p=0.044) and inadequate vitamin B6 (p=0.022) and manganese (p=0.026) intake. Elevated oxidative stress, detected in 33.3% of patients in this sample, was not associated with sarcopenia. Conclusion Most patients in this sample were overweight and sarcopenic. Lean mass depletion was associated with risk of cardiovascular disease, reduced muscle strength, food intake changes, reduced functional capacity, self-reported well-nourished status and inadequate intake of vitamin B6 and manganese, but not with oxidative stress.


RESUMO Objetivo Avaliar os fatores de riscos nutricionais em pacientes pré-transplante de célula-tronco hematopoiética. Métodos Estudo transversal, descritivo, realizado com pacientes de um ambulatório de hematologia. As variáveis estudadas foram demográficas, dados clínicos, avaliação subjetiva global produzida pelo próprio paciente, indicadores antropométricos, ingestão alimentar e estresse oxidativo. Os dados foram considerados estatisticamente significativos quando p<0,05. Resultados A amostra do estudo foi constituída por 72 pacientes, com média de idade de 48,93 (14,5%) anos e com mieloma múltiplo (51,4%) como a patologia mais prevalente. Conforme índice de massa corporal, 55,6% dos pacientes encontravam-se com excesso de peso. De acordo com a circunferência da cintura, índice de conicidade e percentual de gordura corporal, houve prevalência de risco para doença cardiovascular. A sarcopenia foi associada ao risco de doença cardiovascular pela relação cintura/quadril (p=0,021), depleção da força muscular (p<0,001), além da ingestão alimentar (p=0,023), da capacidade funcional reduzida (p=0,048) e do diagnóstico de "bem nutrido" (p=0,044), conforme a avaliação subjetiva global, e com consumo inadequado de vitamina B6 (p=0,022) e de manganês (p=0,026). Dentre os avaliados, 33,3% apresentaram estresse oxidativo elevado sem associação com sarcopenia. Conclusão Pacientes do pré-transplante se apresentam, em sua maioria, com excesso de peso, mas com sarcopenia, estando essa ausência de massa magra associada a risco de doença cardiovascular, depleção da força muscular, alteração da ingestão alimentar, redução da capacidade funcional, classificação de "bem nutrido", segundo a avaliação subjetiva global e consumo inadequado de vitamina B6 e manganês, não estando associada a estresse oxidativo.

Abstract Objective To assess the daily dietary intake and energy contribution of ultraprocessed foods among women who are positive and negative for the human immunodeficiency virus (HIV) during pregnancy. Methods This case-control study included 77 HIV-positive and 79 HIV-negative puerperal women between 2015 and 2016. The socioeconomic and maternal demographic data were assessed, and a food frequency questionnaire (FFQ) adapted for pregnant women was applied. The Fisher exact test and the Mann-Whitney test were applied to detect differences between the groups. Linear regression was used to assess the associations between the intake of ultra-processed food and energy, macro- and micronutrients, with values of p < 0.05 considered significant. Results The HIV-positive group was older (p< 0.001) and had lower income (p= 0.016) and level of schooling (p< 0.001) than the HIV-negative group. Both groups presented similar average food intake: 4,082.99 Kcal/day and 4,369.24 Kcal/day for the HIV-positive and HIV-negative women respectively (p= 0.258).The HIV-positive group consumed less protein (p= 0.048), carbohydrates (p= 0.028) and calcium(p= 0.001), andmore total fats (p= 0.003). Ultra-processed foods accounted for 39.80% and 40.10% of the HIV-positive and HIV-negative groups' caloric intake respectively (p= 0.893). The intake of these foods was associated with a higher consumption of carbohydrates (p < 0.001), trans fat (p= 0.013) and sodium (p< 0.001), as well as lower protein (p < 0.001) and fiber intake (p= 0.022). Conclusion These findings demonstrate that the energy consumption and ultraprocessed food intake were similar in both groups, which reinforces the trend toward a high intake of ultra-processed food in the general population. The intake of ultraprocessed food was positively associated with the consumption of carbohydrates, trans fat and sodium, and negatively associated with the consumption of protein and fiber.


Resumo Objetivo Avaliar o consumo alimentar diário e a contribuição dos alimentos ultraprocessados na dieta de gestantes soropositivas e soronegativas para o vírus da imunodeficiência humana (HIV). Métodos Estudo de caso-controle com 77 puérperas soropositivas e 79 soronegativas entre 2015 e 2016. Analisaram-se dados socioeconômicos e demográficos maternos, e aplicou-se um questionário de frequência alimentar (QFA) adaptado para gestantes. Utilizou-se o teste exato de Fisher e o teste de Mann-Whitney para detectar diferenças entre os grupos. A regressão linear avaliou a associação entre o consumo de ultraprocessados e de energia, macro e micronutrientes. Valores de p < 0,05 foram considerados significativos. Resultados O grupo de puérperas soropositivas foi mais velho (p < 0,001), com menor renda familiar (p = 0,016) e escolaridade (p < 0,001) quando comparado com o grupo das soronegativas. Ambos os grupos apresentaram médias de consumo semelhantes, com 4.082,99 Kcal/dia entre as puérperas soropositivas e 4.369,24 kcal/dia entre as soronegativas (p = 0,258). Observou-se que as puérperas soropositivas consumiam menos proteínas (p = 0,048), carboidratos (p = 0,028) e cálcio (p = 0,001), e mais gorduras totais (p = 0,003). Os ultraprocessados corresponderam a 39,80% das calorias entre as soropositivas, e a 40,10% entre as soronegativas (p = 0,893). O consumo destes alimentos esteve associado a um maior consumo de carboidratos (p < 0,001), gordura trans (p = 0,013) e sódio (p < 0,001), e a um menor consumo de proteínas (p < 0,001) e fibras (p = 0,022). Conclusão Esses achados demonstram que o consumo de energia e de alimentos ultraprocessados foram semelhantes nos dois grupos, o que reforça a tendência ao consumo elevado de alimentos ultraprocessados na população geral. O consumo de alimentos ultraprocessados foi positivamente associado ao consumo de carboidratos, gorduras trans e sódio, e negativamente associado ao consumo de proteínas e fibras.

The consumption of industrialized foods and beverages contributes to the increase in body weight and manifestation of diverse illnesses. Among these industrialized products, soft drinks can be mentioned, which when consumed in large quantities can contribute to weight gain and the emergence of chronic non-communicable diseases such as hypertension, diabetes, and obesity. In terms of performance, several training protocols have been used, among which concurrent training is presented as a tool that can aid in the control of body weight and muscle development. Thus, the identification of dietary factors, as well as the use of appropriate training methods, can be essential factors in the increase in physical performance. To study the effects of concurrent training on the skeletal muscles of animals submitted to soft drink consumption. In the present study, 32 male rats were used, of the Wistar breed, in the post-weaning biological phase (30 days). The animals performed a concurrent training protocol. Forty-eight hours after the final training session, the animals were submitted to a surgical procedure to remove samples of gastrocnemius muscle. After preparation of the histological slides, the muscle fibers of the different groups of animals were measured. The median values for the minimum diameter of muscle fibers presented the following values: Control (C): 35.78, Control Soft Drinks (CSD): 36.93, Training (T): 39.12, and Training Soft Drinks (TSD): 37.82. The CSD and TSD groups demonstrated the greatest increase in body mass in relation to the C and T groups. In addition, the groups that performed the concurrent training presented a higher mean of the smallest fiber diameter. It was concluded that the ingestion of soft drinks had a negative effect on the muscular hypertrophy of animals submitted to a concurrent training protocol.


El consumo de alimentos y bebidas industrializadas contribuyen al aumento de peso corporal y manifestación de diversas enfermedades. Entre estos alimentos, se encuentran las bebidas refrescantes, que al ser consumidas en grandes proporciones, pueden contribuir al aumento de peso y a la aparición de enfermedades crónicas, tales como la hipertensión, diabetes y obesidad. En cuanto al desempeño, diversos protocolos han sido utilizados, entre estos, el entrenamiento concurrente que puede ser una forma de ayuda en el control de peso corporal y el desarrollo muscular. Siendo así, la identificación de los factores alimentarios, como también la utilización de los métodos correctos de entrenamiento, pueden ser esenciales en el aumento del desempeño físico. El objetivo del estudio fue investigar los efectos del entrenamiento concurrente en la musculatura esquelética de animales sometidos al consumo de bebidas refrescantes. En la presente investigación fueran utilizadas 32 ratas Wistar. Los animales fueran sometidos a un protocolo de entrenamiento concurrente. Después de 48 horas de la última sesión de entrenamiento, los animales fueran sometidos a un proceso quirúrgico para la remoción de muestras de tejido muscular. Los valores de la media del diámetro de las fibras musculares fueron: Control (C): 35,78; Control y Bebida Refrescante (CBR): 36,93; Entrenamiento (ET): 39,12; y Entrenamiento y Bebida Refrescante (EBR): 37,82. Se observó que los grupos CBR y EBR presentaron mayor aumento de masa corporal en comparación con C y T. Además, los grupos que realizaran el entrenamiento concurrente obtuvieron una mayor media en el diámetro de las fibras. Es posible concluir que el consumo de bebidas refrescantes tiene una influencia negativa en la hipertrofia muscular de animales sometidos a un protocolo de entrenamiento concurrente.

Abstract Background and objectives: Preoperative fasting may lead to undesirable effects in the surgical patient in whom there is a stimulus to ingesting clear liquids until 2 hours before anesthesia. The aim of this study was to evaluate the gastric emptying of two different solutions using ultrasound. Methods: In a prospective, randomized, blind study, 34 healthy volunteers ingested 200 mL of two solutions without residues in two steps: an isotonic solution with carbohydrates, electrolytes, osmolarity of 292 mOsm.L-1, and 36 kcal; and other nutritional supplementation with carbohydrates, proteins, electrolytes, osmolarity of 680 mO.L-1, and 300 kcal. After 2 hours, a gastric ultrasound was performed to assess the antrum area and gastric volume, and the relation of gastric volume to weight (vol.w-1), whose value above 1.5 mL.kg-1 was considered a risk for bronchoaspiration. A p-value <0.05 was considered statistically significant. Results: There was a significant difference between all parameters evaluated 2 hours after the ingestion of nutritional supplementation compared to fasting. The same occurred when the parameters between isotonic solution and nutritional supplementation were compared 2 hours after ingestion. Only one patient had vol.w-1 <1.5 mL.kg-1 2 hours after ingestion of nutritional supplementation; and only one had vol.w-1 >1.5 mL.kg-1 after ingestion of isotonic solution. Conclusion: This study demonstrated that gastric emptying of equal volumes of different solutions depends on their constitution. Those with high caloric and high osmolarity, and with proteins present, 2 hours after ingestion, increased the gastric volumes, which is compatible with the risk of gastric aspiration.


Resumo Justificativa e objetivos: O jejum pré-operatório pode levar a efeitos indesejáveis no paciente cirúrgico, em que há um estimulo à ingestão de líquidos sem resíduos até 2 horas antes da anestesia. O objetivo deste estudo foi avaliar o esvaziamento gástrico de duas soluções diferentes por meio da ultrassonografia. Métodos: Em um estudo prospectivo, randomizado, cego, 34 voluntários saudáveis ingeriram 200 mL de duas soluções sem resíduos, em duas etapas: uma solução isotônica com carboidratos, eletrólitos, osmolaridade de 292 mOsm.L-1 e 36 kcal; e outra suplementação nutricional, com carboidratos, proteínas, eletrólitos, osmolaridade de 680 mOs.L-1 e 300 kcal. Após 2 horas, fez-se ultrassonografia gástrica com avaliação da área do antro e volume gástrico e relação do volume gástrico sobre o peso (vol.p-1), cujo valor acima de 1,5 mL.kg-1 foi considerado risco para broncoaspiração. Considerou-se p< 0,05 como estatisticamente significativo. Resultados: Houve diferença significativa entre todos os parâmetros avaliados 2 horas após a ingestão de suplementação nutricional em relação ao jejum. O mesmo ocorreu quando foram comparados os parâmetros entre solução isotônica e suplementação nutricional 2 horas após a ingestão. Apenas um paciente apresentou vol.p-1< 1,5 mL.kg-1 2 horas após a ingestão de suplementação nutricional; e apenas um apresentou vol.p-1 > 1,5 mL.kg-1, após a ingestão de solução isotônica. Conclusão: Este estudo demonstrou que o esvaziamento gástrico de volumes iguais de diferentes soluções depende de sua constituição. Aqueles com alto valor calórico e alta osmolaridade, e com proteínas presentes, 2 horas após a ingestão, aumentaram os volumes gástricos, compatíveis com o risco de aspiração gástrica.

ABSTRACT Objectives: The aims of this study are to investigate which of the seven selected predictive equation for estimating basal metabolic rate (BMR) is the best alternative to indirect calorimetry (IC) and to evaluate the dietary energy intake in patients with type 2 diabetes. Subjects and methods: Twenty-one patients with type 2 diabetes participated in this diagnostic test study. Clinical and laboratorial variables were evaluated as well as body composition by absorptiometry dual X-ray emission (DXA) and BMR measured by IC and estimated by prediction equations. Dietary intake was evaluated by a quantitative food frequency questionnaire. Data were analyzed using Bland-Altman plots, paired t-tests, and Pearson's correlation coefficients. Results: Patients were 62 (48-70) years old, have had diabetes for 8 (2-36) yeas, and 52.4% were females. The mean body composition comprised a fat-free mass of 49.8 ± 9.4 kg and a fat mass of 28.3 ± 7.2 kg. The energy intake was 2134.3 ± 730.2 kcal/day and the BMR by IC was 1745 ± 315 kcal/day. There was a wide variation in the accuracy of BMR values predicted by equations when compared to IC BMR measurement. Harris-Benedict, Oxford, FAO/WHO/UNO equations produced the smallest differences to IC, with a general bias of < 8%. The FAO/WHO/UNO equation provided the best BMR prediction in comparison to measured BMR. Conclusion: In patients with type 2 diabetes, the equation of the FAO/WHO/UNO was the one closest to the BMR values as measured by IC.

ABSTRACT Objective: To determine associations between sex and neurodevelopmental outcomes in human milk-fed very preterm infants, adjusted to early measured nutrient intakes and other neonatal cofactors. Methods: Consecutive inborn human milk-fed infants, with gestational age <33 weeks, were eligible. In-hospital energy and protein intakes have relied on measured human milk composition. The Bayley Scales of Infant Development II mental and psychomotor developmental indexes were used to assess neurodevelopment at 20 months' corrected age. After univariate analysis, some covariables were used for linear multiple regression. Results: Thirty-two infants were included, with a mean (standard deviation) gestational age of 29.8 (1.8) weeks, and a median birth weight of 1168g (interquartile range 990-1419g). Minimum recommended intakes were achieved in 63.6% and 15.2% of infants for protein and energy, respectively. The mental and psychomotor developmental indexes were within normal limits in 93.8% of infants. The mean mental and psychomotor developmental indexes were significantly lower in males. Only male sex negatively and significantly affected the mental and psychomotor developmental indexes (B=-9.44; 95%CI: -17.64- -1.23; adjusted r2=0.17; p=0.026), adjusted to gestational age and measured energy intake. Conclusion: In human milk-fed very preterm infants, males had a significantly lower mental and psychomotor developmental indexes score at 20 months' corrected age, adjusted for gestational age and measured energy intake.


RESUMO Objetivo: Determinar a associação entre sexo e desfechos relativos ao neurodesenvolvimento em lactentes muito prematuros e alimentados com leite humano, ajustada para a ingestão de nutrientes medida nos primeiros dias de vida e outros cofatores neonatais. Métodos: Consideramos, para este estudo, lactentes alimentados com leite humano, consecutivamente nascidos em um centro especializado, com idade gestacional <33 semanas. A ingestão intra-hospitalar de energia e proteínas baseou-se na composição medida do leite humano. Os índices de desenvolvimento mental e psicomotor das Bayley Scales of Infant Development II foram usados para avaliar o neurodesenvolvimento na idade corrigida de 20 meses. Após a análise univariada, algumas covariáveis foram usadas para a regressão múltipla linear. Resultados: Incluímos 32 lactentes, com idade gestacional média (desvio padrão) de 29,8 (1,8) semanas e mediana de peso ao nascimento de 1168g (intervalo interquartil: 990-1419g). A ingestão mínima recomendada foi atingida em 63,6% e 15,2% dos lactentes, para proteínas e energia, respectivamente. Os índices de desenvolvimento mental e psicomotor ficaram dentro dos limites normais em 93,8% dos lactentes. A pontuação média nos índices de desenvolvimento mental e psicomotor foi significativamente menor no bebês do sexo masculino. Somente o sexo masculino afetou negativa e significativamente os índices de desenvolvimento mental e psicomotor (B=-9,44; IC95%: -17,64- -1,23; r2 ajustado=0,17; p=0,026), ajustados para idade gestacional e ingestão de energia medida. Conclusão: Em lactentes muito prematuros e alimentados com leite humano, o sexo masculino teve pontuação significativamente mais baixa nos índices de desenvolvimento mental e psicomotor na idade corrigida de 20 meses, ajustada para idade gestacional e ingestão de energia medida.

Abstract Objective: To evaluate the resting energy expenditure, growth, and quantity of energy and macronutrients intake in a group of preterm newborns. Methods: The cohort study was performed with appropriate and small for gestational age preterm infants (birth weight lower than 1500 g or gestational age < 32 weeks). Resting energy expenditure was measured using indirect calorimetry on the 7th, 14th, 21st, and 28th days of life, and at discharge. Length, head circumference and body weight were assessed weekly. Nutritional therapy was calculated during the hospital stay and the information for each type of food was recorded in software that calculates the total amount of energy and macronutrients. Results: 61 preterm infants were followed; 43 appropriate and 18 small for gestational age infants. There was no statistical difference for resting energy expenditure between the groups, and it increased from the first to the fourth week of life (appropriate: 26.3% and small: 21.8%). Energy intake in the first two weeks of life was well below the energy requirement. Conclusion: Considering that the results demonstrate high energy expenditure during the first weeks of life, there is an evident need to provide the best quality of nutrition for each child in the first weeks of life so that preterm infants with or without intrauterine growth restriction can achieve their maximum potential for growth and development.


Resumo Objetivo: Avaliar o gasto energético de repouso, o crescimento e a quantidade ofertada de energia e macronutrientes em um grupo de recém-nascidos pré-termo. Método: Foi feito estudo de coorte com recém-nascidos pré-termo adequados e pequenos para a idade gestacional (peso de nascimento inferior a 1.500 gramas ou idade gestacional < 32 semanas). O gasto energético foi avaliado com a calorimetria indireta nos dias 7°, 14°, 21°, 28° dias de vida e alta hospitalar. Medidas do comprimento, perímetro cefálico e peso corporal foram avaliadas semanalmente. A terapia nutricional foi calculada durante a internação do recém-nascido e as informações de cada tipo de alimentação foram registradas em um software que calcula a quantidade total de energia e macronutrientes. Resultados: Foram acompanhados 61 recém-nascidos, sendo 43 adequados e 18 pequenos para idade gestacional. O gasto energético de repouso não apresentou diferença estatística entre os grupos e aumentou entre a primeira e quarta semana de vida (adequados: 26,3% e pequenos: 21,8%). O aporte energético nas duas primeiras semanas de vida mostrou-se bem abaixo do requerimento energético mensurado pela calorimetria. Conclusão: Considerando os resultados que demonstram um gasto energético alto ao longo das primeiras semanas de vida, fica evidente a necessidade de fornecer ao recém-nascido pré-termo um melhor aporte energético já nas primeiras semanas de vida, para que os neonatos com ou sem restrição intrauterina possam atingir o seu potencial máximo de crescimento e desenvolvimento.

ABSTRACT A total 120 piglets with an average live weight of 7.00 kg, weaned at 21 days, were used to evaluate the effect of neutral detergent fibre levels on the digestibility of nutrients and energy from the diets, productive performance, and the composition and rate of deposition of nutrients and energy in the bodies of piglets in the nursery phase. The animals were distributed according to a randomized-block design into five treatments, which consisted of neutral detergent fibre levels, with six replicates and four animals per plot. A quadratic effect was detected for the digestibility coefficients of nutrients and energy, feed intake and weight gain. The increase in fibre level promoted a linear increase in fat content in the carcass, blood, and body, whereas the energy in the carcass, organs, and body showed an inverse response. The results showed a quadratic effect on the nutrient deposition rate in the carcass, organs and body. In conclusion, the best digestibility of nutrients and energy from the diet is obtained with 10-11.5% neutral detergent fibre, as higher weight gain and greater protein deposition in the body are achieved at neutral detergent fibre levels of 10.6% and 10.3%, respectively.

Introducción: El índice de adiposidad visceral (VAI) es un método sencillo y costo-efectivo en la determinación de adiposidad visceral. El objetivo de este estudio es evaluar la relación entre el VAI con diversos factores de riesgo cardiovascular, variables sociodemográficas y hábitos psicobiológicos en la población adulta de la ciudad de Cuenca, Ecuador. Materiales y métodos: Se realizó un estudio descriptivo transversal en 318 individuos adultos seleccionados mediante muestreo aleatorio y multietápico, a quienes se les realizó evaluación clínica, evaluación antropométrica y de laboratorio. El VAI se determinó utilizando las fórmulas propuestas que emplean circunferencia abdominal, el índice de masa corporal, los triacilglicéridos y HDL-C. Se realizó un modelo de regresión logística múltiple para determinar los principales factores asociados a adiposidad visceral en sus valores más elevados. Resultados: En los 318 individuos, el promedio del VAI fue 2,57 (1,66-3,94), con valores más elevados para el sexo femenino. En el modelo de regresión logística múltiple, los factores de riesgo significativos para VAI moderado-alto fueron: la edad (> 60 años: OR = 3,87; IC del 95%: 1,15-12,96; p = 0,03), el consumo calórico, la glucemia alterada en ayuno y la actividad física en ocio. Conclusión: El VAI es un método útil para definir a aquellos sujetos con adiposidad visceral en nuestra región. La edad, el consumo calórico diario y la glucemia alterada en ayuno son los principales factores asociados con los valores más elevados delíndice, mientras que la actividad física durante el ocio representó un factor protector para clasificar a los sujetos en los estadios más avanzados.


Introduction: The visceral adiposity index (VAI) is a simple and cost effective method for the determination of visceral adiposity. The objective of this study is to evaluate the relationship between VAI and different cardiovascular risk factors, sociodemographic variables, and psychobiological habits in the adult population of the city of Cuenca, Ecuador. Materials and methods: A descriptive cross-sectional study was performed on 318 adult individuals selected by multistage random sampling, who underwent a clinical, anthropometric and laboratory evaluation. VAI was determined using the proposed formula that used abdominal circumference, body mass index, triglycerides, and HDL-Cholesterol. A multiple logistic regression model was used to determine the main factors associated with the highest values of visceral adiposity. Results: The mean VAI was 2.57 (1.66-3.94) in the 318 individuals studied, with higher values for females. In the multiple logistic regression model, significant risk factors for moderatehigh VAI were: age (>60 years: OR = 3.87, 95% CI: 1.15-12.96, P=.03), calorie intake, impaired fasting glucose, and leisure time physical activity. Conclusion: VAI is a useful method to define those subjects with visceral adiposity in our region. Age, daily calorie intake, and impaired fasting glucose are the main factors associated with higher index values, while leisure time physical activity was a protective factor for classifying subjects in the more advanced stages.

ABSTRACT Objective: Thus, the aim of this study was to compare if higher or smaller fibronectin type 3 domain-containing protein 5 (FNDC5)/irisin levels are associated with inflammatory and metabolic markers, caloric/macronutrient intake, physical fitness and type 2 diabetes mellitus (T2DM) risk in obese middle-aged men, and also to correlate all variables analyzed with FNDC5/irisin. Subjects and methods: On the basis of a cluster study, middle-aged obese men (IMC: 31.01 ± 1.64 kg/m2) were divided into groups of higher and smaller levels of FNDC5/irisin. The levels of leptin, resistin, adiponectin, tumor necrosis factor alpha (TNFα), interleukin 6 and 10 (IL6, IL10), lipopolysaccharide (LPS), glucose, insulin, glycated hemoglobin, insulin resistance and sensibility, lipid profile, risk of T2DM development, body composition, rest energy expenditure, caloric/macronutrient intake and physical fitness were measured. Results: The higher FNDC5/ irisin group presented improved insulin sensibility (homeostasis model assessment - sensibility (HOMA-S) (p = 0.01) and QUICKI index (p < 0.01)), insulin (p = 0.02) and triglyceride levels (p = 0.01), lower insulin resistance (homeostasis model assessment - insulin resistance (HOMA-IR) (p = 0.01), triglycerides/glucose (TYG index) (p = 0.02), neck circumference (p = 0.02), risk of T2DM development (p = 0.02), tendency to decrease serum resistin (p = 0.08) and significant lower LPS levels (p = 0.02). Inverse correlations between FNDC5/irisin and body weight (r −0.46, p = 0.04), neck circumference (r −0.51, p = 0.02), free fat mass (r −0.49, p = 0.02), triglycerides (r −0.43, p = 0.05) and risk of developing T2DM (r −0.61, p = 0.04) were observed. Conclusions: These results suggest that higher FNDC5/irisin levels in obese middle-aged men are related to a better metabolic profile and lower risk of T2DM development and serum LPS, a potential inducer of insulin resistance.

ABSTRACT BACKGROUND: Gastrostomy tube feeding (GTF) is indicated for children with feeding difficulties due to tetraspastic cerebral palsy, although there are no definitive conclusions about the benefits of GTF. OBJECTIVE: To compare nutritional status and diet of pediatric patients with tetraparesis cerebral palsy who are fed by GTF with those fed orally (PO). METHODS: A transversal and descriptive study on 54 patients with spastic tetraparesis was held. The referred parameters were: weight, knee height and estimated height, cutaneous folds and circumferences. The Frisancho reference was used to compare the skin folds and body circumferences. The Brooks et al. curve was adopted as a reference for weight, height, and BMI. Food inquiry was performed using the Habitual Dietary Recall method. The total energetic value (TEV) of macronutrients and fibers was performed by Avanutri® version 4.0, a nutrition software program. The differences of nutritional parameters between the GTF and the PO groups were calculated by chi-squared and Fisher's exact tests, and the comparison between the groups for variable numbers was performed using the Mann-Whitney test. The significance level adopted was 5%. RESULTS: The PO group presented more individuals in the malnourished range (24.14%) and high levels of adipose and thin mass depletion. The ingestion of lipids was larger in the GTF group, even though the proteins and fibers were higher in the PO group. The comparison between the diets in the GTF group indicated that the mixed diet (industrialized and artisanal) supplied a greater contribution of proteins and fibers. CONCLUSION: Comparing the groups, the tetraparesis cerebral palsy patients fed orally have a greater impairment of their nutritional status, even though they have higher intakes of protein and fiber than those patients fed by gastrostomy, demonstrating a consistent argument for the use of gastrostomy.


RESUMO CONTEXTO: A alimentação por gastrostomia é indicada para crianças com dificuldades de alimentação devido à paralisia cerebral tetraespástica, embora não haja conclusões definitivas sobre a colocação da alimentação por gastrostomia. OBJETIVO: Comparar o estado nutricional e dieta de crianças e adolescentes com paralisia cerebral tetraespástica alimentados via oral (VO) e via gastrostomia. MÉTODOS: Estudo transversal e descritivo de 54 pacientes com paralisia cerebral tetraespástica. Os parâmetros aferidos foram: peso, altura do joelho e estatura estimada, dobras cutâneas e circunferências. Curvas de Brooks et al. foram adotadas como referência. O inquérito alimentar foi realizado pelo método Recordatório Alimentar Habitual. As diferenças dos parâmetros nutricionais entre os grupos Gastrostomia e VO foram calculadas por testes qui-quadrado e Exato de Fisher e a comparação entre os grupos para as variáveis numéricas foi realizada pelo teste de Mann-Whitney. O nível de significância adotado foi de 5%. RESULTADOS: O grupo alimentado via oral apresentou mais indivíduos na faixa da desnutrição (24,14%) e maior depleção de massa magra e adiposa. A ingestão de lipídeos foi maior no grupo Gastrostomia, enquanto que de proteínas e de fibras foi superior no grupo VO. A comparação entre as dietas no grupo Gastrostomia indicou que a dieta mista (industrializada + artesanal) fornece maior aporte de proteínas e fibras. CONCLUSÃO: Os pacientes com paralisia cerebral tetraespástica alimentados por via oral têm um maior comprometimento do estado nutricional, mesmo que eles tenham um maior consumo de proteínas e fibras que os pacientes alimentados por gastrostomia, demonstrando um argumento consistente para o uso de gastrostomia.

Abstract INTRODUCTION: Nutritional status has been implicated in the modulation of the immune response, possibly augmenting the pathogenesis of Chagas disease (Cd). We evaluated diet quality and nutritional status in adults and elderly patients with chronic Cd in a tertiary hospital. METHODS: A case-control study of Cd patients was conducted, paired for gender, age, and co-morbidities with non-Cd patients. Anthropometric measurements and food frequency questionnaire was used, and diet quality was assessed by the Brazilian Healthy Eating Index-Revised (BHEI-R). The Estimated Average Requirement cut-off points were used to determine the dietary micronutrient adequacy. The Cd group was further grouped according to Los Andes classification. RESULTS: The study participants were 67 ± 10 years old, 73.6% elderly and 63% female. The prevalence of overweight/obesity and abdominal fat was high in both groups; however, Cd group showed a lower prevalence of obesity and increased risk of disease according to waist circumference classification. There was no difference in BHEI-R score between groups (p=0.145). The Cd group had sodium and saturated fat intake above recommendations and low intake of unsaturated fat, vitamin D, E, selenium, magnesium, and dairy products; but higher intake of iron. According to Los Andes classification, group III presented lower intake of whole fruit and dietary fiber. CONCLUSIONS: Patients with Cd were overweight and the quality of their diet was unsatisfactory based on the recommended diet components for age and sex.

ABSTRACT The use of animals from dairy farms is an alternative to meat production since it provides an increment of total income for farmers. This study aims to evaluate the performance of Holstein calves finished in two feeding systems (feedlot or pasture). Forty-three animals with 58 days old and 57 kg were divided in two treatments: 23 animals finished in feedlot with corn silage plus concentrate based on corn and soybean meal (40:60); 20 animals kept in cultivated pastures according to the period of the year: Italian ryegrass (Lolium multiflorum) and pearl millet (Pennisetum americanum) with supplementation with the same feedlot-concentrate at 1% body weight. Animals were slaughtered with 200 kg. Dry matter and nutrient intake were determined, with the use of chromium oxide for estimating pasture intake. Feedlot animals had greater total intake and total digestible nutrients, resulting in higher average daily gain (0.949 vs 0.694 kg day-1). Crude protein intake, neutral detergent fiber and feed conversion did not show significant differences. Holstein calves have improved performance when finished in feedlot.

ABSTRACT

The study of the factors that regulate high energy food intake is especially relevant nowadays due to the high prevalence of overweight and obesity. Food intake regulation can be divided in two basic processes, namely satiation and satiety. Satiation is the process that determines the moment in which feeding stops and regulates the amount of ingested food during a single meal. Satiety is the interval between meals and regulates the time elapsed between two meals. The longer the interval, the lower energy intake. Each of these processes are regulated by different factors, which are here reviewed.

Abstract: Objective: To evaluate food intake according to the degree of processing, stratified by family income and age, in a representative sample of children younger than 6 years in the city of Pelotas, RS, Brazil. Methods: Cross-sectional population-based study carried out with 770 children aged 0-72 months of age living in the urban area of Pelotas. The dietary intake of children was assessed by 24-h recall administered to mothers or guardians. The energy intake was estimated and each food item was classified according to the food processing degree. Food consumption was stratified by age (younger than 24 months; 24 months or older) and associations between quintiles of family income and relative contribution of each food to total energy were performed by linear regression. The Wald test was applied to test linear trend across groups. Results: The mean energy intake was 1725.7 kcal/day. The mean contribution of processed and ultraprocessed foods was 19.7% among children younger than 24 months and 37% in those aged 24 months or older, while the mean consumption of natural and minimally processed food was 61% and 44%, respectively. Among children aged 24 months or older, a greater consumption of canned foods, cheese and sweets was observed as family income quintiles increased, while breads were more consumed by those children belonging to the lower income quintiles. Conclusion: A high caloric contribution of ultraprocessed foods in detriment to a lower consumption of natural and minimally processed foods was observed in the diet of children younger than 6 years.


Resumo: Objetivo: Avaliar o consumo alimentar conforme o grau de processamento, segundo a renda e a faixa etária, em uma amostra representativa de crianças menores de 6 anos de Pelotas (RS), Brasil. Métodos: Estudo transversal conduzido com 770 crianças até 72 meses residentes na zona urbana de Pelotas. O consumo alimentar das crianças foi avaliado por recordatório de 24 horas, aplicado às mães ou aos responsáveis, e o consumo calórico dos alimentos foi estimado de acordo com o grau de processamento. O consumo alimentar foi estratificado por faixa etária (menos de 24 meses; 24 meses ou mais) e as associações entre renda familiar e a participação relativa dos alimentos no total de calorias diárias foram conduzidas por regressão linear simples. Fez-se o teste de Wald para avaliar tendência linear entre os grupos. Resultados: A média de consumo foi 1.725,7 Kcal/dia. A participação calórica proveniente do grupo de alimentos ultraprocessados foi de 19,7% nas crianças com menos de 24 meses e 36,1% naquelas com 24 meses ou mais, enquanto que a contribuição do grupo de alimentos in natura e minimamente processados foi de 61,2% e 44,1%%, respectivamente. Nas crianças com 24 meses ou mais observou-se maior consumo de doces conforme o aumento da renda familiar, enquanto que os pães foram mais consumidos entre as crianças pertencentes aos menores quintis de renda. Conclusão: Observou-se elevada participação calórica de alimentos ultraprocessados em detrimento do menor consumo de alimentos in natura e minimamente processados na alimentação de crianças menores de 6 anos.
